How does home care work?

An assigned caregiver will visit the patients’ homes to provide the care and assistance they need every day, from morning to night. The amount of assistance given will vary depending on the needs of the patient.

Prior to receiving care, an in-home assessment will be conducted for our team to identify and recognize the needs of the patient. Our case manager will then create a care plan suited to the care requirements and preferences of the patient at home.

The individualized care plan will be re-assessed and updated over time as the needs of the patient change.

What are the types of services offered?

Companion Care

This type of care focuses more on providing companionship to a patient at home. A companion sitter/caregiver will visit the patient to check up on him or her and provide assistance when needed.

Some services provided:

  • maintain schedule and reminders
  • medication reminders
  • transportation to doctors’ appointments
  • run errands

Light Housekeeping

A caregiver assists with basic household chores. We understand that due to aging or physical limitations, a patient may no longer be capable of doing chores at home. This is why our caregiver will help the patient maintain a clean and healthy home.

Some services provided:

  • laundry
  • sweeping and mopping
  • dishwashing
  • organizing furniture

Personal Care

Assistance with Activities of Daily Living (ADLs) is one of the most important support our caregivers can provide to our patients. A caregiver offers assistance with personal hygiene and other self-care tasks.

Some services provided:

  • assist getting out of bed
  • monitor vitals
  • meal planning and preparation
  • bathing, dressing, and grooming
  • toileting
